Assertive Community Treatment (ACT)

For individuals with Severe Mental Illness

Assertive Community Treatment (ACT) is an evidence-based practice that improves outcomes for people with severe mental illness (SMI) who are most at-risk of: homelessness, psychiatric hospitalization,
and institutional recidivism. ACT’s goal is to give consumers adequate community care and to help them have a life that isn’t dominated by their mental illnesses.

Types of Services

ACT services are delivered by a multidisciplinary team of providers who conduct assertive outreach in the
community where the consumer lives. Services may include:

  • Therapy
  • Case management
  • Peer support
  • Medication services
  • Housing assistance
  • Employment assistance

How does ACT Help Consumers?

ACT provides assertive community outreach to the consumer to provide support while learning to implement independent living skills and symptoms management skills to assist in decreasing the need for
crisis services, hospitalization, and legal involvement related to their mental health. ACT Strives to help consumers become self-sufficient and improve their overall quality of life and engagement within the
community.

To Access the ACT Program

Schedule an intake assessment, by calling your local Pawnee office and let the admissions staff know that an intake is needed. If you already are receiving Pawnee services and believe that you qualify for this program, please contact your provider and request a referral.
